Two prices for the same pen
Most UK pharmacies list a new-customer price and a returning-customer price for each GLP-1 medicine. GLP Index records both. The first order price is what you pay once. The ongoing price is what you pay for every supply after it. The difference is the pharmacy's cost of winning you, and it is paid back from month two.
The gap is not fixed. On 21 October 2026 the cheapest Mounjaro 5 mg first order was £120.00 against a cheapest ongoing price of £125.00. For Foundayo 0.8 mg the same two figures were £46.00 and £79.99. At the cheapest pharmacies the picture differs because the cheapest first order and the cheapest ongoing price are often at different pharmacies. At Wegovy 2.4 mg, for example, the cheapest first order was £112.00 at Medicspot (GPhC 9012559), and the cheapest ongoing price was £139.00 at Molecule (GPhC 9011284). Who the gap affects
Anyone paying privately in England, Scotland, Wales or Northern Ireland. There is no first order price on an NHS prescription: the charge is £9.90 per item in England and nothing in the other three nations, every time. The NHS weight loss injections guide sets out who qualifies.
What a year costs once the first order price has gone
Take Mounjaro and a schedule that follows the SmPC minimum of four weeks per step: one pen each at 2.5 mg, 5 mg, 7.5 mg, 10 mg and 12.5 mg, then seven pens at 15 mg. The first order discount comes off the first pen only. The other 11 pens are at the ongoing price, so the ongoing price sets almost all of the year's cost.
The choice of pharmacy moves the total far more than the first order offer. Twelve pens of Mounjaro 15 mg cost £2,808.00 at the cheapest ongoing price and £3,599.88 at the median, a difference of £791.88.
Wegovy follows the same pattern. A schedule of one pen at each of 0.25 mg, 0.5 mg, 1 mg and 1.7 mg, then eight at 2.4 mg, also has one discounted pen and 11 at the ongoing price. Twelve pens at 2.4 mg cost £1,668.00 at the cheapest ongoing price and £2,435.40 at the median.

Steps to budget for month two onwards
- Find the ongoing price at your current dose on the price page for your medicine. Ignore the first order column.
- Find the ongoing price at the next dose up. If your prescriber steps you up, this is next month's bill.
- Multiply the ongoing price at the dose you expect to settle on by the number of months you plan to stay on it.

Mistakes that cost money
Reading the first order price as the monthly price. Choosing a pharmacy on the first order price when its ongoing price sits at the median or above. Treating a 3, 6 or 12 month plan as a way to keep the first order rate, when the plan is priced at the ongoing rate and locks you in.
