Hospital trip costs: travel, parking and overnight stays

NHS hospital treatment is free, but the trip around it often is not. In England the Healthcare Travel Costs Scheme refunds fares and fuel if you hold an HC2 or HC3 certificate or get a qualifying benefit, with 3 months to post form HC5(T). NHS hospital parking is free in Scotland and Wales. English trusts must park four groups free, Blue Badge holders among them. Charity houses beside children's hospitals charge families nothing.

The costs around free treatment

The NHS Constitution for England says NHS services are free of charge, apart from limited exceptions Parliament has approved. A consultant appointment, a scan or a week on a ward costs you nothing. The spending happens on either side of the care: bus and train fares, fuel, hospital car parks, a companion's ticket and a bed for a parent who lives too far away to go home each night. A course of weekly treatment repeats those costs every week.

Refunds for travel

England's Healthcare Travel Costs Scheme repays the cheapest reasonable cost of reaching hospital or specialist care you were referred to. Entitlement comes through a benefit such as Pension Credit Guarantee Credit, through Universal Credit with net earnings of £435 or less in the latest assessment period, or through an HC2 or HC3 certificate from the NHS Low Income Scheme. Many hospitals pay out at a cashier's office on the day. Where none does, you post form HC5(T) within 3 months. Scotland and Wales use their own HC5 forms. Northern Ireland pays cash at the hospital.

Parking

Your nation decides the bill. NHS hospital sites in Scotland and Wales do not charge for parking. Trusts in England set their own tariffs, yet must let Blue Badge holders, frequent outpatients, parents of children kept in overnight and night-shift staff park free. Northern Ireland still charges, and a law passed in May 2026 fixes 12 May 2029 as the latest end date.

Transport for people who cannot travel alone

Non-emergency patient transport is NHS-funded travel for patients whose medical condition or mobility makes other travel unsafe. NHS England's criteria from May 2022 say most people should get to hospital by their own means.

Somewhere to sleep

Ronald McDonald House Charities UK runs 14 Houses next to children's wards and charges families nothing. Young Lives vs Cancer runs 10 Homes from Home on the same free basis. Hospital relatives' rooms carry a nightly price: Queen Elizabeth Hospital Birmingham charges £45 for a single.

Questions people ask

How long do I have to claim hospital travel costs back?

In England a postal claim on form HC5(T) must reach the right office within 3 months of the date you paid. A later claim needs a written explanation, and the NHS Business Services Authority decides whether the delay had a good reason. In Northern Ireland the limit is three months after the appointment.

Can my family claim travel costs for visiting me in hospital?

No. The NHS website lists visiting someone in hospital as a journey the Healthcare Travel Costs Scheme does not cover. An escort's fares are refunded only when a healthcare professional says it is medically necessary for someone to travel with the patient, and the claim then rests on the patient's eligibility.

Is there free accommodation for parents of a child in hospital?

Yes, through charities. Ronald McDonald House Charities UK runs 14 Houses beside children's wards and charges families nothing, with a hospital referral needed. Young Lives vs Cancer runs 10 free Homes from Home in 8 cities for families of children and young people having cancer treatment.

Can I get money for hospital travel before the appointment?

Sometimes. The NHS website says you are normally expected to pay and claim back within 3 months, but an advance payment may be available in some cases. Ask the service treating you or your local integrated care board before you travel.
